Clinicians must be well informed on how best to treat cellulitis, abscesses and other cutaneous infections in order to reduce the need for systemic antibiotics. In my extensive practice of medicine, Methicillin-resistant Staphylococcus aureus (MRSA) infections are infrequent and usually avoidable. The most common error that I see in Medicine is that providers often fail to readily incise and drain an abscess. The clinician must consider the bacterial milieu associated with a specific area in order to select the most appropriate antibiotic. In this area, the bacteria consist of mixed aerobic (Escherichia coli, Klebsiella pneumoniae, Staphylococcus aureus) and anaerobic (Bacteroides species, Clostridium, Veillonella) flora. I have treated many perianal abscesses via incising over the fluctuant area and draining the purulent material. I have usually prescribed the first generation cephalosporin, Cephalexin. Before incising, I apply anesthesia, lidocaine 2% with epinephrine, over the roof of the abscess, epidermal layer, using a very small needle, 27G. Do not use an anesthetic with epinephrine for digital blocks (fingers; toes). If the patient is allergic to penicillin and the cephalosporins, the antibiotic choices become limited to a macrolide (erythromycin) or trimethoprim-sulfamethoxazole (TMP-SMX; BactrimÂ®, SeptraÂ®) or Vancomycin for MRSA. I rarely prescribe TMPL-SMX to otherwise healthy patients (non-HIV) since it has the highest incidence of adverse side effects including angioedema, urticaria (hives), thrombocytopenia (low platelets), and rarely exfoliative dermatitis, Steven-Johnson syndrome. As reported in the British media, a woman, after taking TMP-SMX for sinusitis, barely survived after "losing her skin." This woman was in the hospital for six months and made a 'miraculous recovery.' An abscess may often be successfully treated by simply performing an I&D (Incision and Drainage). Doctors often endanger a patient and take unnecessary medical-legal risk, as was the case of the patient with the left gluteal abscess in the video. This patient had a lumbar epidural (spinal) steroid injection for his chronic back pain a day before I saw him despite his already having the readily visible abscess. Though he was suing his company for chronic back pain, his pain had nothing to do with his job. Once I helped him resolve complex issues, his back pain went away. The usual bacteria involved in Impetigo, cellulitis and skin abscesses are typically Streptococcus pyogenes, Staphylococcus aureus, Streptococcus pneumoniae, group G or C streptococci and rarely gram negatives like Escherichia coli. In neonates (newborns), the clinician should consider Group B streptococci as a potentially causative agent. Treatment for cellulitis in a child younger than 5 years should provide coverage also for Haemophilus influenzae. For children and adults, I usually prescribe Cephalexin, Dicloxacillin or a macrolide like Erythromycin estolate or Erythromycin ethylsuccinate (quicker acting than Azithromycin). An ingrown toenail with an abscess or a bacterial paronychia with an abscess must be readily incised and drained on the first visit. The patient with the Noonan Syndrome was over four years old when I first saw him. His condition had gone undiagnosed though he had been born the United States and had had his usually immunizations.
